comprehensive primary care for children, youth and adults. It’s a health
care setting that facilitates partnerships between patients, their personal
physicians, and when appropriate, the patient’s family.

Webinar of the Month
Each month the MaineHealth PCMH website will highlight a webinar brought
to you by Community Care of North Carolina. These webinars offer an
in-depth look at recognition standards, review of the NCQA workbook, the

Introduces the Patient Centered Medical Home concept, describe the benefits
of practice recognition and improvement, and describe the tools and
documentation used in the application process. It will also address the

                How can my practice apply for NCQA recognition?
